November 2018. The Miami Herald had already published its exposé on Epstein's plea deal. His crimes were public knowledge. The emails continued.

Epstein introduced Dr. Harry Fisch — a New York urologist and officer of the pharmaceutical company Veru — to Boris Nikolic, then managing director of Biomatics Capital and previously a top science advisor to Bill Gates.

The commercial purpose. Fisch's company had merged with the Female Health Company, manufacturers of the FC2 Female Condom. Fisch was seeking investment and specifically hoped Nikolic could introduce him to the Gates Foundation for grants to distribute female condoms across Africa.

The Uganda number. The email chain cites the shortage explicitly: "Uganda: 3 million units" — the quantity requested and not supplied.

Boris Nikolic. Nikolic appears elsewhere in the files in deeply troubling contexts. His correspondence with Epstein extends back years and includes grossly inappropriate exchanges. He was also named — reportedly without prior knowledge — as a backup executor in Epstein's will. And yet as late as November 2018 he was receiving business introductions from Epstein.

The female condom introduction was not isolated. The files document Epstein's sustained positioning inside the Gates philanthropic apparatus over nearly a decade:

• 2011 — JPMorgan Chase executives were emailing Epstein for advice on structuring a philanthropic fund for Bill Gates
• 2013 — Gates "specifically requested" that Epstein serve as representative to his own science advisor, citing their "existing collegial relationship" — five years after Epstein's conviction
• September 2014 — as Ebola was declared a threat to international peace and security, Gates was updating Epstein on a meeting with President Obama
• EFTA01038315 — Epstein actively managing which science topics Joichi Ito would pitch to Gates through MIT's Media Lab

Why this matters more than the dollar amounts. If Epstein was curating which scientific topics and researchers reached Gates through the MIT conduit, he held influence over a philanthropic pipeline that dwarfs his own direct funding capacity — the Gates Foundation deploys billions annually into global health, much of it in Africa.

The necessary caveat. Children's Health Defense's own analysis of these documents concluded: "The architecture exists. The orchestration does not." The files establish Epstein's presence inside global health financing design. They do not establish that he directed its outcomes.

The unnamed associate known as "MD" — the woman at the centre of the 2010 Kampala cluster — wrote to Epstein in November 2009 that she spent "10K per year supporting medical student associations in Nepal, China, Uganda and Tanzania."

The same document establishes her as a Gates Foundation-connected figure with a neurosurgery career, who also curated a children's bedtime story collection including Ugandan folklore.

What this reveals. A second, parallel philanthropic channel through which Epstein's network engaged African medical education — separate from the Fisch–Nikolic commercial introduction, operating through personal giving rather than institutional grants.

The control dynamic. By August 2014 the same woman was writing to Epstein: "Can I go to Uganda and Zimbabwe? I don't need a visa.." He replied: "yes to 505." A neurosurgeon funding medical student associations across four countries required his permission to travel.

Identifying her remains the single most consequential outstanding task for any journalist pursuing the Africa files.

The pattern is not African. Epstein's medical and scientific access operated identically in Cambridge, Manhattan, and Kampala — and the institutional failures that permitted it were overwhelmingly Western.

Mount Sinai. A $50,000 grant from a convicted sex offender to Dr. Jess Ting, eight years after conviction, during the period he became director of America's first comprehensive academic transgender surgery programme. When asked in 2026 what due diligence was performed, a spokesperson said only: "We've encouraged Dr. Ting to respond directly."

MIT. $7.5 million recorded as anonymous. The director resigned. The review did not examine the influence agenda.

Harvard. Larry Summers took leave in 2024 over Epstein ties. Epstein funded science across the university for years post-conviction.

Weill Cornell. The Cornell Daily Sun documented gifts, dinners, and deals with multiple Weill Cornell doctors in February 2026.

NPR's assessment (March 2026): private philanthropy provides an estimated 10–20% of university research funding, with little legal or public scrutiny. Epstein used this to cultivate scientists and launder his reputation. There is still no universal reporting system for these gifts.

Epstein's African health involvement produced no documented medical outcome — no confirmed grant, no delivered supply, no completed programme. What it produced was position.

He sat between a pharmaceutical company and the world's largest health funder. Between a neurosurgeon's personal philanthropy and four countries' medical student bodies. Between MIT's director and Bill Gates' attention.

In each case the value was not the transaction. It was being structurally necessary to it.
